Direct Debit	Set up direct debit for regular contribution payments. Complete a Direct Debit Request Form (Form 16) Mail it to us at Vision Super, PO Box 18041, Collins Street East, Melbourne VIC 8003
Cheque	Pay contributions by cheque. Complete Remittance of Contributions Form (Form 39) Attach a cheque payable to Vision Super Post it to: Vision Super, PO Box 18041, Collins Street East, Mebourne VIC 8003
