Choice of fund - Form 1b (120.06 kb) This form should be given to all new employees within 28 days of joining your company if Vision Super is your default fund.
Payroll deduction agreement - Form 13d (65.04 kb) This form should be given to any employees that want to make additional contributions to their super account via payroll deductions. This form stays with the employer and is for internal use only.
Service fraction alteration - Form 26a (46.04 kb) Use this form to inform us of a change to a defined benefit members service fraction. A service fraction relates to how many hours an employee works per week.
Super Saver top-up account - Form 29b (48.21 kb) Use this form to establish a Super Saver top-up account for a defined benefit member who wants to make additional contributions. This will open a Super Saver account for those contributions.
Secure website access - Form 50a (79.34 kb) This form should be used by employers who want to access the secure section of the website. Contribution files can be sent via the website.
Authorised officer - Form 50b (79.38 kb) This form should be filled in by an employer to authorise an employee to use the website. This employee then has the power to authroise other staff members to use the website.
Clearing house application - Form 54 (148.88 kb) This form should be used if an employer would like to use the clearing house. The clearing house allows employers to make payments to multiple super funds in one streamlined process.
Contribution payments
Direct debit request - Form 16 (71.97 kb) Use this form if you want to make contribution payments for your employees via direct debit. After we receive the form we can then debit your bank account for the contributions when you direct us to do so.
Cheque and EFT - Form 39 (36.71 kb) Use this form if you are making contributions payments by cheque or EFT and are not submitting your contriubtions via the website. This allows us to match up your contribution payments with your employees.
