Direct debit request - Form 16 (71.97 kb) This form should be filled in with Form 12 if you want to make extra contributions to your super account via direct debit from your bank account.
Spouse contributions - Form 13a (59.31 kb) Use this form to make contributions into the Partner Plan on your spouse`s behalf. Note this form should only be used if you had a spouse account open prior to 01/02/2007.
Payroll deduction agreement - Form 13d (65.04 kb) Use this form to make voluntary contributions out of your salary. This form should be given directly to your payroll department so they can make the contributions from your salary.
Early release of benefits due to hardship - Form 60 (119.82 kb) This form should be filled in if you want to apply for access to your preserved benefit on the grounds of financial hardship. There are rules about when a preserved benefit can be released.
Contribution splitting - Form 61 (71.33 kb) Use this form if you want to split some of your contributions with your spouse. This form explains which contributions can be split.
Family Law Regulation 72 Notice - Form 72 (52.68 kb) Under family law, a person can receive information about their spouse`s superannuation when going through a divorce. This form should be used if you are in this situation.
